CPFT are excited to be able to offer a part - time (18.75 hours) permanent position for a Band 5 Occupational therapist with their North Early Supported Discharge Service for Stroke.
This Service forms a key part of the pathway for those surviving stroke and their family/carers. We support people to leave hospital more quickly and provide intense Neuro rehabilitation in the community for a six week period to enable patients to maximise their recovery and independence as quickly as possible.
We are seeking an enthusiastic and dynamic band 5 Occupational Therapist to join the North area Stroke Early Supported Discharge Team. This role will be a job share with our current band 5 and will support the delivery of the Early Supported Discharge (ESD) Service for Stroke with in-reach into the community beds, as well as work with patients in their own homes.
Support will be available from within the multi-professional Stroke ESD team and also from the Community Neuro Rehab teams who are co-located on site with the Community beds. Our teams consist of Occupational therapists, Physiotherapists, Speech and Language therapists, Psychologists, Stroke Co-ordinators and Neuro Rehab assistants.
This service has been developed in conjunction with North West Anglia NHS Foundation Trust (NWAFT) & Cambridge University Hospitals NHS Foundation Trust (CUH) and is part of an exciting new model of care built around the patient in the community. You will play a vital part in supporting patients with physical, cognitive and psycho-social challenges to achieve their maximum potential.

Support is provided both clinically and Operationally and there will be the opportunity to attend training and develop yourself professionally within this exciting post.
Post holders will work in an agile fashion and have access to our ESD bases in the North Locality (Peterborough and East Cambs and Fenland)
